Always use the most current Version ("v" number of the report.)

NAME: CDBG Reports

DESCRIPTION: Aggregated data for the quarterly CDBG report for FY11-12  

INTERESTING FACTS: This report filters by AMI (MFI) of 80% or less and by Eugene and Springfield zip codes.  This report provides an unduplicated count of client ID numbers and HH IDs. The report includes entry/exit responses for each client ID so the user can easily find and correct errors in ServicePoint. The agency needs to select the correct FY12-13 for their Provider(s).

DESCRIPTION: These reports show all the required fields on the summary and the actual data entered on the detail. Export to excel.

INTERESTING FACTS: These reports count Entry/Exits- not people.

 

NAME: Active Entry with No Service Since 30 60 90 180 Days LC.V2

LOCATION: Public Folder/Lane County/Active Entry with No Service Since 30 60 90 180 Days LC.V2

DESCRIPTION: Use this to determine which clients have not been provided a service in XX number of days. Purpose is to assist providers in identifying clients to exit because they are no longer receiving services. Export to excel.

INTERESTING FACTS: This reports looks at open ENTRY/EXITS (no exit date) and looks at the last service transaction given by the ENTRY/EXIT Provider. It generates 4 lists: 1) List of clients who have not had a service in at least 30 days, 2.) List of clients who have not had a service in at least 60 days, 3.) List of clients who have not had a service in at least 90 days, 4.) List of clients who have not had a service in at least 180 days. This report is used to determine who to exit from programs due to inactivity.

 

NAME: Client Demographics LC V1

Formerly called CSBG Demographics lns v5

DESCRIPTION: Client and Household demographics such as race, gender, ethnicity, age groups, income levels, veteran status and more.

INTERESTING FACTS: This report counts new and continuing clients in the date range who have and entry/exit entry in the selected provider(s).

 

NAME: Current Bed Inventory -0341-v11.04.13

Inventory available (occupied and unoccupied) in your Permanent Supportive Housing, Transitional Housing for Homeless, or Emergency Shelter Program. This is what we report to funders on the capacity of housing for homeless in our community. Run it for a specific date. Inventory changes with added/reduced funding, etc.

 

NAME: HUD CoC APR -0625-v12 - BETA LC V1

LOCATION: Public Folder/Lane County/

DESCRIPTION: The Annual Performance Report (APR) for SHP and SPC projects in the HUD CoC grant. But you can run on any case management program to look at entry/exit comparison outcomes. 

INTERESTING FACTS: This report is a copy of HUD CoC APR -0625-v12 – BETA which is in the Continuum of Care folder. One of the performance measures has been corrected in the Lane County version.

DESCRIPTION: Provides a list of all Services available to select in the dropdown list sorted by Provider Name.

DESCRIPTION: Listing the other services at other provider programs which clients in the selected programs have received. Example: Of Provider A Food Pantry’s 500 clients 15 also received rent assistance from Provider B and 27 received transportation assistance from Provider C.
