Voter Registration Forms

To Obtain a Registration Card:

• If you are military, a military dependent, or a US citizen temporarily residing overseas, click on Federal Registration Card .
• All other Lane County Residents click on  Oregon Voter Registration Card  .

After completing the appropriate Voter Registration Form, mail it directly to the Lane County Elections office or county in which you reside. The Lane County Elections Office address is 275 W 10th Avenue, Eugene OR 97401.

A voter who is unable, because of a disability, to sign their name should also complete the  Signature Attestation form.
