OPUS Reports

Agencies using OPUS may request the following reports.
Note: Each agency has one or more OPUS leads that are able to request the report. OPUS Leads have active OPUS user accounts and are identified as having permission to request reports.

• APR Detail

  Description: Client ID, Household ID, Applicant Type, Enrollment Date, Exit Date, last service transaction, last housing transaction, number in household, gender, age at enrollment, HUD CoC enrollment assessment date, household (family) type, food stamps, msfw, HUD HH type (family or single), gender, age, veteran, chronic homeless, ethnicity, race, mental illness, Alcohol abuse, drug abuse, HIV/AIDS, developmental disability, physical disability, domestic violence, verified disability, prior living situation, household entry at enrollment, household entry at exit, length of stay in program, reason for leaving
  Type : This report is in Excel.
  Use : Use this report to review details on the aggregated Annual Progress/ Performance Report (APR).
  Request: Provide the OPUS Program name (APC) and the date range of your APR.
   
• Time to Exit

  Description : Client ID, program enrollment date, last service transaction, last housing transaction, race, ethnicity, prior residence type, current address, and CoC enrollment assessment date if applicable. Report sorted by enrollment date.
  Type : This report is a PDF.
  Use : Use this report to determine who should be exited from your program based on date of last service or enrollment date if no service recorded.
  Request: Provide the OPUS Program name(s) (APC).
  
• Data Review

  Description : Fields include OPUS User Login Name, email address, Client ID, program enrollment date, Last zip code, household type, household size, prior residence type, CoC Enrollment assessment (if applicable), client demographic details, service transactions, housing transactions.
  Type : This report is a PDF.
  Use : This report is for OPUS users to review the data they entered into OPUS in the prior week or month to check for data quality and completeness.
  Request: Provide the OPUS USER LOGIN NAME and the date range .
   
• Client Enrollment and Exit Detail
  Description : Client ID, program enrollment date, exit date (if applicable), last service transaction and OPUS intake worker ID, last housing transaction and OPUS intake worker ID. Report sorted by All Open Clients and all Closed Clients in order of enrollment date.
  Type : This report is a PDF.
  Use : Use this report to determine who should be exited from your program based on date of last service.
  Request: Provide the OPUS Program name(s) (APC).
   
• Enrollment Data
  Description: Client ID, Household ID, Applicant Type, Enrollment Date, Exit Date, last service transaction, last housing transaction, zipcode at time of enrollment, number in household, gender, age, ethnicity, race, education, health insurance, verified disability, veteran, household type, household size, poverty level, homeless, food stamps, msfw, prior residence, destination residence, household income at enrollment, and household income at exit.
  Type : This report is Excel.
  Use : Use to verify that the enrollment data is being entered and entered correctly.
  Request: Provide the OPUS Program name(s) (APC).
   
• Services Summary and Detail

  Description :  List of all services, number of households receiving it, number of individuals in the households who received it, dollar value per household, and number of units (for example: days, dollars, or hours.)  Optional : Followed by the detail of each household with all individuals and the services received in the date range. This report is run for a date range (for example: 01/01/08 to 12/31/08)

• Housing Transaction Summary and Detail

  Description :  List of all housing transactions, number of households receiving it, number of individuals in the households who received it, dollar value per household, and number of days. Optional : Followed by the detail of each household with all individuals and the housing transactions received in the date range. This is run for a date range (for example: 01/01/08 to 12/31/08)

  Request : Provide the OPUS Program name(s) (APC) and the date range.
   
• Food Box Report Summary and Detail

  Description: Summary of new households who received a their first food box in the fiscal year in the report month and continuing households who received a food box in the month. Includes the count of households, adults, children, and the number of food boxes. Optional : Followed by an alphabetical listing of the households and household members who received the foodboxes. This report is run for the prior month's food box service transactions.

  Use : This report provides data for the monthly food box report.
  Request: Provide the OPUS Program name(s) (APC) and the date range.
   
• CDBG Eugene and Springfield

  Description: Data for the CDBG Quarterly reports for Eugene and Springfield including number of households and individuals in the quarter, race/ethnicity, median household income, female head of households, and disabled. This report is generated from service transactions "CDBG- EUGENE ENROLLMENT" and "CDBG- SPRFLD ENROLLMENT" with the start date in the quarter's date range.

  Use : This report shows the detail of the households receiving food boxes for a data entry check. It also provides data for the monthly food box report.
