Personal Support Worker (NEW)

To become a new PSW you must start by completing the required forms, scheduling a New PSW appointment and submitting them to our office at the time of your appointment.

Personal Support Worker (RENEWAL)

If you are within 120 days from your credentials expiring it may be time for you to being the process for your Criminal History Check.  It is the sole responsibility of the PSW to be aware of expiration dates and to act on it accordingly.

PSW background checks can take up to 12 weeks to clear and starting the process early will help you from experiencing a delay in your pay. If your PEAA and/or CHC lapses before you start the renewal process, then the period in between your expiration date and when the renewal is approved will NOT be billable.

To start the renewal process please complete the required forms, schedule a PSW Renewal appointment and submitting the documents to our office at the time of your appointment.

If you are an existing PSW or Employer of Record and want to add a client or PSW the PSW-Domestic Employee Form needs to be filled out and submitted to our office.

FAQ's

Q: Is there a cost to me for this background check?

A: No, however, If you provide us with information that is a qualifier for fingerprints; i.e. out of state residence in the last 5 years or any criminal history, fingerprints may be required. If fingerprints are required, you will be instructed to schedule an appointment for Livescan with a Third Party agency. You must be prepared to pay $12.50 by credit/debit card at the time that you schedule this appointment. Appointments for fingerprints should be made within two days of being notified that you need fingerprints.

Q: What will I need to bring with me to this appointment?

A: For ALL Background Checks SI’s must bring a valid state issued photo ID, for most it will be a driver’s license but other acceptable forms include state issued ID, passport, social security card, or others (like birth certificate or military ID).

Q: How long do my credentials last once I am approved?

A: If the state approves your background check then you may work for two years as a paid provider. If you begin fulfilling other job duties than which your current background check is approved for, you may be required to fill out paperwork for a new background check.

Other licensed or certified professional that are considered Subject Individuals (i.e. Behavior Consultant, Behavior Professionals,  CNAs, etc.) will need to schedule an in-person appointment with Lane County DDS.

Other Important Information

A less than spotless background check may not disqualify you to work as a paid provider for Lane County.

Presenting fraudulent information on your Criminal Background check may disqualify you from working as a paid provider for Lane County.

If the State declines to approve your Criminal History check then you may not work as a paid provider for Lane County.
