FUSE Housing and Case Management Services

Lane County, through the Department of Health & Human Services, is seeking respondents to a Request for Qualified Providers to provide housing and case management to single adults experiencing homelessness who experience Severe Mental Illness (SMI) or co-occurring SMI and Substance Use Disorders.

In order to be considered a Qualified Provider, respondents must have capacity to provide all of the following:

1. Provide emergency shelter for 10 targeted individuals at any point in time.
2. Provide rapid re-housing for 13 targeted individuals at any point in time.
3. Place 10 targeted individuals in housing via Section 8 disabled homeless preference vouchers.
