Phone Appointments

Your phone appointment may require the following items that can be emailed: [email protected]

  • Eligibility Proofs: ID/Income/Address for you and your child(ren).  Links below for examples.

  • HIPAA Receipt Acknowledgement: Please read.  Complete & sign bottom section.  Email back (only needed if brand new to WIC).*

  • Rights & Responsibilities: Read the Rights & Responsibilities. 

  • Participant Signature Form: Complete and email back.*

  • Height, weight, and iron test results (HGB): You can use information from a doctor visit or a home scale and measuring tape.

  •  Voter Registration Opportunity: Click on link to sign up.    

* If you are unable to fill out the Participant Signature and HIPAA forms, please send an email along with your proofs stating: "I have read and agree to the participant rights and responsibilities and have received HIPAA information."

WIC is working remotely at this time.  This means calls from us may appear as blocked, restricted or unknown. 

For scheduled appointments, our calls will come between the scheduled time and up to 15 minutes afterwards.  For example, if you are scheduled for a 10:00am appointment, your WIC call will come between 10:00am and 10:15am.  We will try to reach you twice during that time period.

Once staff reach you, your appointment may last from 25-40 minutes per person.
