What is Coordinated Entry

Coordinated Entry is a system-wide coordination for a more effective and strategic response to homelessness. It works to ensure people experiencing homelessness or at risk of homelessness can access the most appropriate resources through a standardized assessment, prioritization, and referral process. It works to align housing resources into this process so that the resources a household can access are not dependent on where they access services.

Process of Coordinated Entry

  • Diversion – Problem solving conversations to help identify if there is a way to immediately resolve the situation, or connect to available resources
  • Assessment – Collect additional information to help with prioritization
  • Prioritization – Prioritization for resources based on identified needs, strengths, and vulnerabilities
  • Referral – Connect to different types of housing supports, as spaces become available
