> HOUSING

  • 63.5% of HHs transition from emergency shelter to more stable housing (transitional or permanent)
  • 71.5% INDs reside in permanent housing for over 6 months
  • 71.5% INDs reside in permanent housing for over 7 months
  • 80% of all HHs remain housed upon exit from Homeless Prevention Program
  • 65% of the INDs receive services and develop self reliant skills which enabled them to exit to permanent housing
  • 92% of youth sheltered returned home or moved into safe, alternative housing

  • 50% of SPMI INDs upon exit will have obtained SSI, SSD, food stamps, veterans or other mainstream benefits

> MENTAL HEALTH

  • 30% of youth receiving pre-treatment services enter treatment
  • 50% of youth provided treatment reduced or eliminated substance abuse within one year from their enrollment date
  • 100% of clients seeking services become aware of options and available community resources
  • 50% of INDs were connected with community services to address their clinical condition and needs as a direct result of engagement with outreach workers
  • 20% of INDs completed applications for Safe Haven Shankle or other supportive housing facility or day shelter.
  • 10% of INDs were assisted in getting services at Safe Haven Shankle or other supportive housing facility or day shelter
  • % of INDs receiving appropriate disposition for crisis mental health issue

> KNOWLEDGE/SKILLS

  • 90% will demonstrate enhanced success factors, e.g. increased adult supervision, quality communication, conflict management, resource use, or school enrollment.
  • 100% of the INDs upon exit will have received supportive services (case management, transportation assistance, etc) to help them access services, create stability, meet goals and develop greater self determination
  • 60% of parents will complete the parenting education series and develop the following skills: see plan
  • 100% of families will identify needs and become more aware of available resources as a result of their involvement with Direction’s supportive service program
  • 50% of parents increased competence, ability to manage stress, knowledge of child development and reduced feelings of isolation
  • 75% of HHs exited program completed their self-development programs
  • 50% of HHs enrolled in Open Doors will attend Birth to Three or other parent education classes
  • 50% of INDs enrolled in the class will complete the series and gain knowledge and skills of the following: see plan
  • 75% of group INDs will identify new skills to increase their safety

 

> CASE MANAGEMENT

  • 80% of HHs served will not experience an incident of domestic violence while receiving services
  • 95% of all HHs remained intact (no foster care placement for the children)
  • 75% of all children enrolled per year will not require abuse or neglect reports to DHS
  • 75% of the children exiting to public school did not require special education intervention
  • 100% elderly INDs receiving in-home services were enabled to remain independent in their own home
  • 95% of the teen HHs will not experience a subsequent pregnancy
  • 90% of ex-offenders will exit transitional housing without having committed a new crime or having been reincarcerated
  • 71.5% of the ongoing caseload will have length of stay in program over six months

 

> CUSTOMER SERVICE

  • 80% of behavioral health INDs will return for additional services
  • 80% of behavioral health INDs identified the service as helpful
  • 20% of youth and families calling will receive increased level of service.

 

> ENERGY SERVICES

  • 16%  decrease in the number of  noncompliant payment occurrences during and after the year after program completion
  • %10 decrease in annual energy use by households participating in energy conservation and case management programs
  • 5% decrease in the annual energy use by households participating in the low income weatherization program

 

> VETERAN SERVICES

  • 66% of veteran services applications result in benefits.
