NEW PATIENT REGISTRATION PACKET

Community Health Centers of Lane County has made it easier for first time patients to establish care by offering several convenient options for obtaining a new patient registration packet. Choose one of the following options:

Online Registration:
Simply use one of the buttons below to complete and submit your registration online!

Registration packet on site:
Stop by one of our clinic locations for a new patient registration packet. See all clinic locations and hours of operation. Complete forms on site or take home to fill out at your own pace. Please return forms to the clinic of your choice.

Registration packet via mail:
Call us at 541-682-3550 and press option 2 to speak to a customer service representative to ask for a registration packet to be sent to you in the mail. Please return forms to the clinic of your choice.

Download and print a registration packet:
Select the adult or pediatric option and print the packet at home. Please return completed forms to the clinic of your choice.

Adult New Patient Registration Forms

1. Print adult registration packet and complete at your own pace.
2. Drop off the completed packet at any of our clinic locations.
3. Staff will schedule your new patient appointment when they receive your forms.

Pediatric New Patient Registration Forms (ages 0-18)

1. Print pediatric registration packet and complete at your own pace.
2. Drop off the completed packet at any of our clinic locations.
