How do we define family?

A family is a group of people who come together to invest in the next generation, to care for pregnant and postpartum people, and to surround babies and children with folks who are ready and able to help them thrive.

What is the Family Connects Oregon program?

Family Connects Oregon is an in-home nurse visit available at no cost to ALL families in Lane County with a new baby, including adoptive parents, families who have other children, families with any kind of health insurance (including OHP), or those with no health insurance.

After you bring your newborn home, we can come and weigh them, support around feeding and lactation, and answer all your questions that might have come up between pediatrician visits. We can visit you any time before your baby turns 12 weeks old.

 


It’s our mission to make life a little easier for parents of newborns—so we come to you, at no cost to you, to make sure you have everything you need to care for yourself, your baby, and your whole family.

What is the Nurse-Family Partnership program?

Nurse-Family Partnership connects you early in your pregnancy with your own personal Registered Nurse. They will provide the support, advice, and information you need for a healthy pregnancy, a healthy baby, and to be the best parent you can be. They can also help you connect to other resources, like diaper banks, healthcare, or even getting back to school or your career after baby arrives.

What is the Healthy Families program?

Healthy Families Oregon partners you with a professionally trained Family Support Specialist either during your pregnancy or in the first 3 months of your baby's life. They meet with you wherever you'd like, and provide resource connection and navigation, parenting education, and support in whatever you want most- going back to school or work, juggling multiple children's needs, or managing your stress.

Just found out you're pregnant?

Not sure where to go for support? We can help with that.

If you are pregnant and need health insurance, we can connect you to the Oregon Mother's Care program, which helps you complete an Oregon Health Plan (OHP) application and get you approved faster than if you applied on your own. OHP prioritizes all pregnant clients and they are processed much more quickly.

We can help get you connected to medical care, whether with a midwife or OB.

We will connect you to WIC and SNAP for food vouchers and nutrition education for a healthy pregnancy and healthy baby. Connection to other community resources and services is also available.

Where should I start?

The fastest way to get support from Family & Child Health is to fill out our intake form. Once you submit the form, a member of our team will reach out as soon as we can, usually within 3 business days. Explore our page to learn more about our various programs.

How long are the visits?

Personal Nurse Visits

  • Typical nurse visits last 1-1.5 hours, all in the comfort of your home.
  • Family and caregiver education and skill building on how to be the best parent you can be.
  • Nurse Case Management including referrals & support to access resources tied to your family’s goals.

Family Support Specialist Visits

  • Typical visits last 1-1.5 hours, all in the comfort of your home.
  • Family Support Specialists empower and support parents in building nurturing relationships with their child, learn about child development, and access resources in the Lane County community.

Common Terms and Links

Can you help me get housing?

We will support you in navigating available community resources towards housing, including application support and building your self-advocacy skills, but we do not have special access to housing directly.

My child needs home health services. Can you help?

Family & Child Health does not offer home health services, such as tracheostomy, IV, ostomy, or wound care. Please contact your health insurance company or CCO for support in accessing those services: Trillium Member Services at 1-877-600-5472.



This health center receives HHS funding and has federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

LCPH services are available regardless of age, race, color, sex, religion, national origin, physical or mental disability, sexual orientation, gender identity, and marital status. LCPH facilities are wheelchair accessible. LCPH materials are available upon request in alternative formats such as: large print, Braille and other languages.
