Continuum of Care

The Continuum of Care (CoC) Program is funded through the Department of Housing and Urban Development (HUD) and is designed to promote communitywide commitment to the goal of ending homelessness; provide funding to quickly rehouse homeless individuals and families while minimizing the trauma and dislocation caused to homeless individuals, families, and communities by homelessness; promote access to and effect utilization of mainstream programs by homeless individuals and families; and optimize self-sufficiency among individuals and families experiencing homelessness.


The OR-500 Eugene, Springfield/Lane County Continuum of Care encompasses the full expanse of Lane County. Lane County Human Services Division is the Collaborative Applicant and Continuum of Care Lead Agency. In this role, Lane County is responsible for overseeing the work of the CoC, as well as applying for and securing over $4 million in annual funding to support housing programs and services operated through Co-Applicants St. Vincent de Paul and Homes for Good, and various other non-profit service providers in Lane County. The work of the CoC is governed by the Poverty and Homelessness Board and is carried out by a wide variety of stakeholders including non-profit providers, business community, people with lived experience of homelessness, healthcare providers, government partners, and community advocates, among others.


Continuum of Care funding primarily supports Permanent Supportive Housing (PSH) programs, Rapid Re-housing (RRH) programs, Coordinated Entry, the Homeless Management Information System, and System Planning.

NOTICE: As of December 8th, HUD has withdrawn the FY25 HUD CoC and YHDP Notice of Funding Opportunity in order to make edits and revisions. HUD intends to re-release this NOFO at a later date. We do not have any information regarding HUD’s timeline for doing so.

 

Lane County will continue with the established local competitive process for new and renewal projects. Once projects are selected, reviewed, and ranked, the competition will pause until HUD releases the updated NOFO. Please check back here for additional updates.

 

New Project Opportunity – HUD CoC Competition


New Projects
 are selected through a Request for Proposals process. If interested in applying for a new project, please review the RFP Opportunity.


This opportunity is now available. Lane County Department of Health & Human Services is seeking proposals from providers to fund Transitional Housing, Supportive Services Only, and Street Outreach Supportive Services Only projects through one or multiple service providers. Funding available is estimated to be $5,105,395, including $3,738,686 reallocated from existing projects and $1,366,709 in Bonus funding.


Proposals are due by 12:00 pm (PST) on December 8, 2025.


The goal of the Youth Homelessness Demonstration Program (YHDP) is to support the development and implementation of a coordinated community approach to preventing and ending youth homelessness and sharing that experience with and mobilizing communities around the country toward the same end. The population to be served by the demonstration program is youth ages 24 and younger who are experiencing homelessness, including unaccompanied and pregnant or parenting youth.

 

YHDP New Projects are selected through a Request for Proposals process. If interested in applying for a project, please review the YHDP RFP Opportunity.

 

Lane County Department of Health & Human Services is seeking proposals from providers to fund Supportive Services Only – Diversion and Kinship project. Total HUD YHDP funding available under this RFP is estimated to be $255,918. One or multiple projects or proposals may be funded by this RFP.

 

 

Proposals are due by 1:00 pm (PST) on December 8, 2025.




Following submission, the CoC Committee will review all proposals and determine which projects will move forward to submit an application to HUD. Project funding is dependent on HUD award (anticipated May 2026).

HUD CoC Renewal Project Local Competition

 

On behalf of the CoC Committee of the Poverty and Homelessness Board (PHB) and the Continuum of Care, Lane County is announcing the release of the 2025 Supplemental HUD CoC Local Project Renewal Form. This form is to supplement the already completed Intent to Renew and Project Review process that concluded in June. All FY24 HUD CoC funded PSH and YHDP funded Joint TH-RRH, SSO- Outreach projects expected to renew in FY25 must complete the form for each project by the stated deadline in order to have the project application ranked in the competition. HMIS, DV CE-SSO, and Planning Projects do not have to complete this form.

 

If you have received a notification that your project is being fully eliminated in the FY25 competition through the Reallocation process, you do not need to submit a form for that project. If you are unsure, please contact Amanda Borta at [email protected]

New Projects

This opportunity is now available. Lane County Department of Health & Human Services is seeking proposals from providers to fund homeless services for domestic violence survivors through one service provider:

Joint Transitional Housing or Rapid Re-housing Domestic Violence (DV) Project is anticipated to be funded up to $130,000 (CoC) for one-year (grant period to begin July 1, 2025) to serve no less than six (6) households at any given time.

Proposals are due by 12:00 pm on August 15, 2024


The CoC Committee of the Poverty and Homelessness Board has voted to dedicate all other new funding in FY24 to Coordinated Entry Supportive Services Only (CE-SSO) activities to support the implementation of the Coordinated Entry System and to expand the Bridges on Broadway PSH project selected in FY23 with any remaining funds. Renewals are evaluated through the Intent to Renew and Evaluation process (local competition). The Intent to Renew and Evaluation process is now open as of April 24, 2024 and is due no later than 5pm on May 17, 2024.
