Why is the Brookside Clinic closing?

Like many health care organizations, we ae experiencing rising costs and reduced funding, To continue providing high-quality care while operating more efficiently, Community Health Centers of Lane County has made the decision to consolidate services.

When is the last day of service?

The Brookside Clinic will close after Friday, November 7, 2025

Where are the providers moving?

Providers will be moving to the Charnelton Community Clinic and the West 11th Clinic.

Where will I see my provider?

Letters have been mailed out to patients with information about their new location. Contact us if you haven’t received this letter.

What happens to my future appointments?

If you already have an appointment on or after November 10, it will automatically be updated to the new clinic location. You do not need to take any action.

Will my medical records transfer automatically?

Yes. Your medical records will move with your provider, so your care will continue without interruption.

How do I get to the new clinic?

Our website www.lanecountyor.gov/chc has an interactive map with addresses and bus routes for all CHC clinic locations. You can also ask our front desk for a map and parking information for the Charnelton Community Clinic and West 11th Clinic.

What if I want to be seen at a different clinic?

You may continue your care at any of our other CHCLC clinic locations. Our staff can help you find the one most convenient for you.

Where can I find information about the other clinic locations?

You can visit our website at www.lanecounty.org/chc for clinic addresses, hours, and directions. Staff are also happy to provide details.
